Health Forms

To participate in Athletics, each student must fill in an Emergency Form and have had a physical within the last year. If that Physical is older than 60 days, but less than one year, a student must fill out a Health History form.

• Health History Form - may be completed by the parent/guardian
• Physical Form - must be filled out by a physician
• Sports Emergency Form - must be filled out each season by a parent/guardian
• Emergency Information Card - must be filled out at the beginning of each school year
• Medication Authorization Form - must be filled out by both parent/guardian and physician

Physical Requirements

To properly care for the health and safety of your child and to meet the requirements of DePaul Catholic High School/Wayne Board of Health, a complete annual physical examination is required of all students each year and is good for one full year (365 days) for both school/field trips and sports requirements.

Your child will not be allowed to enter this school if these forms are not in place by the first day of school in September. Therefore please mail, fax or personally deliver these forms to our health office as soon as possible.

Parents/Guardians must fill out:

• Part A: Health History Questionnaire of the Annual Athletic Pre-Participation Physical forms. Please note the last page (NJSIAA Steroid Testing Policy) does not need to be signed if your child is not participating in a sports activity. (This form must be submitted every time a student wishes to try-out for any type of sports activities).
• Part B: Physical Evaluation of the Annual Athletic Pre-Participation Physical forms must be completed by your child’s physician. Many doctors will ask that Part A is filled in and submitted with Part B before he/she will finalize Part B. We recommend that you make a copy for your own records.

If a student also wishes to participate in sports activities, an additional form is required and can also be downloaded from our website. This form is called "Sports Emergency Form" and both sides should be completed before returning.

All necessary forms must be submitted at least one week prior to tryouts/practice (whichever comes first) or clearance cannot be guaranteed in time for participation.

Individual Health Care Plans

Students who have medical problems that might need nursing intervention and special teacher consideration need to have IHCP on file for each school year. Parents need to view these plans with their medical doctor and return them signed and dated quickly. Changes can be made in these plans at any time. Students who had medical conditions needing an IHCP last year, need to update the plan for this school year. An IHCP can be initiated at any time during the year, as new medical needs arise.
