The main goal of the Health Office is to ensure the overall well-being of each student as well as maintain their safety. In order for me to properly prepare for each school year, the following documentation is required:
Immunization Records – All new/transfer students must provide a copy of their immunization record prior to their first day of school. For returning students, if you have received any recent immunizations, please provide an updated copy of your immunization record as well.
School Physical Requirements- Annual physicals are a prerequisite for all students and not just students who are playing sports therefore, all students must have a current physical on file. As a reminder, physicals are good for one full year (365 days) from the date they were performed. (If your child is playing sports, please ensure that you keep copies for the online registration process).
Action Plans - if they apply, are:
Asthma Treatment Plan
Allergy/Epi-Pen (FARE) Plan
An updated Action Plan MUST be completed by both the physician and parent/guardian at the beginning of each school year.
Medication Administration Forms – Students are not permitted to carry medication while at school, and as a reminder, the school does not keep stock medication on hand. If you anticipate your child needing medication during school hours, a medication form must be completed by both the physician and parent/guardian. A new medication form must be completed for each school year.

Yearly Health Screenings are conducted as follows:
Height, Weight, and Blood Pressure (for all grades),
Vision (8th&10th grade)
Hearing (11th grade)
Scoliosis (9th &11th grade ONLY),
Please fill out the Scoliosis form ONLY if your child is in 9th or 11th grade.

Stand Tall is DePaul Catholic High School's voluntary drug testing program.
Families may download, fill out, and sign the form below, and return it to Ms. Dana Sevean (seveand@dpchs.org).
