IMMUNIZATION REQUIREMENTS
All new/transfer students must present a copy of their immunization record upon registration and/or prior to their first day of school. For returning students, if you have received any recent immunizations, please provide an updated copy of your immunization record as well. Any student who has not received the state mandated immunizations will be excluded from school.
Physical Examination Requirements for All Students
All students are required to have a current physical on file in the Health Office. Annual physicals are a prerequisite for all students and not just for students who are playing sports. As a reminder, physicals are good for one full year (365 days) from the date they were performed. (If your child is playing sports, please ensure that you keep copies for the online registration process). Your child may be asked to return home if these forms are not in place by the first day of school in September.
Parents/Guardians and Physicians must fill out:
- Parent Portion: PRE-PARTICIPATION – HISTORY FORM must be completed by the parent.
- Physician Portion: PREPARTICIPATION PHYSICAL EVALUATION – PHYSICAL EXAMINATION FORM must be completed by the physician.
- Physician Portion: PREPARTICIPATION PHYSICAL EVALUATION MEDICAL ELIGIBILITY FORM must be completed by the physician.
INDIVIDUAL HEALTH CARE PLANS (IHCP) and/or ACTION PLANS
IHCP - Students who have specific medical diagnoses that might need nursing intervention and special teacher consideration may need to have an IHCP in place. Parents need to review these plans with their medical doctor, and return them, signed and dated each school year. An IHCP can be initiated at any time during the year, as new medical needs arise. Changes can also be made in these plans at any time if needed with documentation from the physician only. As a reminder, IHCP plans do not continue from one school year to another, therefore they must be renewed annually and dates must reflect the current school year.
Action Plans - Some students may be required to have an Action Plan in place. If your child has a Diagnosis of Asthma, Seizure or an allergy requiring an Epi-pen, then they are required to have an Action plan in place. Action Plans must be completed and signed by both the physician and parent/guardian. As a reminder, Action Plans do not continue from one school year to another, therefore they must be renewed annually and dates must reflect the current school year. Please know that Action Plans are not only limited to Asthma, Seizure and Epi-pen. Forms for Asthma and Epi-pen can be found on the school website.

MEDICATIONS
Students are not permitted to carry any medications during school hours. This includes Tylenol, Advil, Tums, Benadryl, etc. If you anticipate your child needing medication for headaches, cramps, body aches, or other discomforts, a Medication Form must be completed by both the physician and parent/guardian. The Medication Form can be found on the website. No phone consents will be accepted. As a reminder, Medication Forms do not continue from one school year to another, therefore they must be renewed annually and dates must reflect the current school year.
As a reminder, the school does not keep stock medication, therefore all medications must be brought to the Health Office. Medications must be clearly labeled and in an original container. Any medication that is kept in the Health Office must be picked up at the end of the school year. If not picked up, then it will be properly discarded.
TO REPORT AN ABSENCE
To report an absence for all grades, call 973-694-3702, then press #201.
INJURIES AND ILLNESS
All medical notes related to injuries must be submitted to the health office within 48 hours of the injury.
All medical notes related to illness/absence must be handed into the Main Office and must be received within two weeks of the doctor visit.
Post-dated notes will not be accepted as a medical excuse after that time.
HEALTH SCREENINGS
Health Screenings will be conducted throughout the school year. We expect to begin the screenings in September.
Annual Health Screenings are as follows:
Height, Weight, and Blood Pressure (for all grades),
Vision (8th&10th grade)
Hearing (11th grade)
Scoliosis (9th &11th grade ONLY),
Please fill out the Scoliosis form ONLY if your child is in 9th or 11th grade.
All 12th grade students will receive copies of their immunization records at the end of their senior year.

Medication Administration Forms – Students are not permitted to carry medication while at school and as a reminder the school does not keep stock medication on hand. If you anticipate your child needing medication during school hours, a medication form must be completed by both the physician and parent/guardian.
