The main goal of the Health Office is to ensure the overall well-being of each student as well as maintain their safety. In order for me to properly prepare for each school year the following documentation is required:
Immunization Records – All new/transfer students must provide a copy of their immunization record prior to their first day of school. For returning students, if you have received any recent immunizations, please provide an updated copy of your immunization record as well.
School Physical Requirements- Annual physicals are a prerequisite for all students and not just students that are playing sports therefore all students must have a current physical on file. A parent/guardian must complete the History Form and the Physician must complete the Physical Examination Form. As a reminder, physicals are good for one full year (365 days) from the date they were performed. (If your child is playing sports, please ensure that you keep copies for the online registration process).
Action Plans - if they apply, are:

Allergy/Epi-Pen (FARE) Plan
An updated Action Plan MUST be completed by both the physician and parent/guardian at the beginning of each school year.
Medication Administration Forms – Students are not permitted to carry medication while at school and as a reminder the school does not keep stock medication on hand. If you anticipate your child needing medication during school hours, a medication form must be completed by both the physician and parent/guardian. A new medication form must be completed for each school year.

Hearing (11th grade)
Scoliosis (9th &11th grade ONLY),
Please fill out the Scoliosis form ONLY if your child is in 9th or 11th grade.

DePaul Van Transportation Information
The towns listed below have historically been served by the DePaul Van/ Bus.
Bloomfield, Montclair, Prospect Park, Newark, Orange, East Orange, Rutherford, Lyndhurst, Kearny, Nutley, Belleville, Garfield
We hope to add the following towns this year.*
Wanaque, Haskell, Ringwood, Kinnelon, Oak Ridge, Butler, Bloomingdale, Riverdale, West Milford, Hawthorne, North Haledon, Haledon
*This will depend on ridership numbers.
If you reside in one of the towns listed above, and are interested in utilizing a DePaul Van/Bus this year, please submit a $100 deposit.* Vans only accommodate 14 students, so they fill up quickly.
The cost of one-way transportation for the 2023-2024 school year (to school only) is $1,600. The $100 deposit will be deducted from the $1,600 ($1,500). Once you reserve your spot on the van, you are billed for the entire year. No daily charges. Parents are responsible for after school transportation home.
If we are unable to run your route, or your route becomes “full” before you are able to submit your fee, your $100 deposit will be applied to your tuition payments.
Seats on the van/bus are awarded on a first come/first served basis.
If you live in a town close to the one listed above, you may sign up for the van. Stops are general and not door to door.
